I use this information solely to conduct DOT SAP evaluations under 49 CFR Part 40, guide you through the Return-to-Duty (RTD) process, communicate with you about appointments, and ensure FMCSA Clearinghouse compliance.3. Disclosure of Information:
Your records are protected under HIPAA and 42 CFR Part 2 (substance use disorder records). I will never sell your information. I am legally required to disclose specific records to your Designated Employer Representative (DER) and the FMCSA Clearinghouse as required by federal DOT regulations. I may also disclose to authorized treatment providers with your signed consent.4. Data Security:
I use secure Google Forms to collect information, and data is stored securely in accordance with federal healthcare regulations.5. Your Rights:
